We are glad you have chosen our center to place your child. We are proud of our program and feel confident that you are choosing the right facility.

There is a certain amout of paperwork we must maintain for our records. When your child is accepted for enrollment into our program, it is your responsibility to provide a developmental history.

We must have a Health Status Report. There is a place on the enrollment card for you or your doctor to sign stating your child is free of all communicable disease.

Additionally, we must have a copy of your child's immunization record. All records must be updated annually.
